Groin and anterior femoral pain is relatively common, especially amongst women, and can sometimes be debilitating. These symptoms are often wrongfully diagnosed as generalized diagnoses such as pelvic girdle syndrome, or as fibromyalgia. It can also be misdiagnosed as meralgia paresthetica, femoral or saphenous neuralgias. This article will detail an unknown but common musculoskeletal pathology...

I made this short infomercial regarding the seminars, with a sneak peek at some principles from the low back and sacroiliac joint pain sections. One important thing I forgot to mention was that low back pain that only occurs with lumbar EXTENSION can be a fairly reliable sign of foraminal stenosis. Lumbar foraminal stenosis is different from cervical foraminal stenosis seeing as it is almost ex...

A seemingly common but, at least to the best of my knowledge, unknown musculoskeletal injury mechanism, is infraspinatus impingement at the subacromial passage. The superior portion of the infraspinatus passes partially under acromial roof (the acromial bone), and in internal rotation of the humerus, an even greater amount of the infraspinatus tendon will be exposed to potential impingement at ...

Costoclavicular compression is a wildly common cause of chronic chest, arm, neck and upper back pain, but is not well known. It is also the most important corrective in neurogenic thoracic outlet syndrome. Moreover, sadly, it is monstruously mistreated with cues such as "lower your shoulders", or worse, "pull your shoulders back and down". This worsens costoclavicular impingement and inevitably...

I wrote an article back in 2017 called "Are you a chronic muscle clencher? Woe to you!". And, although it may be the least appreciated piece on my website, it is probably the most important one. Back in 2015 or 2016, I noticed what seemed to be an obvious connection between very high-strung patients and their propensity to having lots and lots of MSK problems, regardless of being seemingly youn...

The solution for dysfunctional neck postures is not to pull the neck back, as the patient is already hyperextending the neck due to neck "hinging". I strongly recommend to stop this detrimental therapeutic approach. There are three main factors to good neck posture, in my opinion. 1., a decent extension of the upper thoracic spine (applies mainly when being upright. Not as important when sittin...

The levator scapulae are frequently vilified for being culprits of pain, compensated and dysfunctional scapular movements, and "hyperactivity". The reality is that the levator scapulae are important stabilizers of the cervical-, and especially upper cervical spine segments. It also stabilizes the scapula, prevents depression of the collarbone during appropriate anterior tilting (eg., during hig...

Faulty scapular upwards rotation is undoubtedly the most common cause of chronic shoulder impingement syndrome and scapular dyskinesia. Patients who are able to perform gentle upwards rotation in posture are generally able to correct their scapular dyskinesis relatively quickly. However, patients who are not able to perform correct muscle activation tend to struggle heavily to correct their pro...

The pectineus and its brethren (add. brevis and longus) are common culprits of anterior thigh pain, quad weakness, anterior testicular and labial pain in patients with tight groins, as it causes a problem I have named pectineo-femoral pinch syndrome, which is positional compression of the nerves that emerge under the inguinal ligament. Groin tightness is common in patients with high stress (due...

I am not a big fan of stretching in general, as most tense muscles tend to be weak. However, the pectoralis minor muscle is a fairly common exception to the norm. I don't recommend the typical doorway stretch because 1. it pulls the clavicle down and back, compressing the costoclavicular space and thus worsening symptoms for TOS patients, and 2. Because it can make certain TOS patients with CVH...

Weakness of the wrist flexors is common. Many, many athletes have weak wrist flexors despite having very strong grips, and this can compromise wrist stability and lead to increased proneness to injury of the wrists. Weakness of the wrist flexors may also predispose to entrapment of the ulnar nerve within the cubital tunnel, as the FCU's fascia hardens with muscular dysfunction, as shown by Stec...

5:49 I am wondering if you could clarify about the long neck cue/pulling the back of the head up as the nose comes forward. How do we pull the back of the head up? Is it using some muscle in the neck? Is it sort of “stretching” the head upwards so that spot raises? I’m a little confused about the cue and am wondering if you could elaborate. Thank you so much for all of your content!

Amazing. Thank you for explaining. I've had a clicking throat when I swallow for the past 6 months (and a sensation of tightness / narrow gullet for several years before this). I'm also experiencing trouble with swallowing / food getting stuck in the throat, often. I have to twist my neck to the side in order to get it down! I recently had a scan that ruled out anything sinister - thank goodness. What you say totally makes sense. I have an old neck injury, and I must be holding my head/neck/jaw in a way that helps to "stabilise" against the old injury too. I have been told in the recent past that my muscles around the front of the neck and shoulders are tight. I hold a lot of tension. I am aware of it myself. When I do bother to make myself mindful of my posture, I realise that I'm holding my jaw clenched - probably almost all the time. I also tend to hold my arms and shoulders tight too. It's something I'll have to work on releasing from now on, that's for sure! I'm sure anxiety and stress play a large part in it too. No doctor I've seen has discussed this with me. A chiropractor was the only person who got near mentioning that I hold tension here. But didn't go into the detail of the muscles involved, like what you have done. Thank you again for your time and explanation. I will try the exercise, and I feel positive about the results to come!!

More likely that the jaw and facial symptoms have led to you relaxing your face more, which causes droopiness of the facial muscles. It's not pathology per se.

usually goes along with quad clenching. Has nothing to do with weakness in the psoas; the latter being a symptom of the postural issue and habits rather than a cause.

Going to give this a shot, thanks a lot! Pulled my levator scapula 9 months ago in ju jitsu (guy had 20 kg on me trying to crank the f*ck out of my neck and I didn't want to tap). Have never had neck issues, anytime I had a sore next would always be fine after a bit. This time it seems I pulled my levator scapula and it's been bothering me since, pain runs down along my trap and even driving is a little bit bothersome. Going to give this a shot and see how it goes. Physio was useless, told me to work neck extensors and it would solve itself (my neck extensors are stronger than 99% of the population in all likelihood)...

Been benching scap back and down since the early 90’s. Pre 90’s I could not bench more that 3 plates. Since 90’s till now benching 4 and a half plates. I’m now 60 and still benching heavy with no shoulder issues. If I did bench flat my shoulders would have been mush by now. The secret is use the larger muscles to lift not the smaller ones inside the shoulder. Take advice from a lifetime strong natty bencher in his 6th octet.

I can make it significantly louder by sending my lower jaw out past my upper teeth (creating an underbite).
